Case of the Week (week 46, 2024)

A 69 year-old woman, with no personal or family history of breast or ovarian cancer, referred for routine screening mammography.

The breasts are dense type C according to the BI-RADS classification.

There was some hesitation around an area of architectural distortion that was very difficult to visualize on 2D imaging without the help of AI on a single incidence: this image could have gone unnoticed in the patient flow.

MammoScreen however immediately spotted this anomaly on the 2 incidences with a high suspicion score of 9.

A tomosynthesis and ultrasound scan are performed at the patient’s arrival, confirming the presence of an attenuating mass and irregular contours classified as BI-RADS 5 at the union of the external quadrants of the right breast, at 9h 3 cm and measuring 8x7x8 mm.

Ultrasound biopsy confirms that it is a grade 3, RH+, Her2-, non-specific infiltrating carcinoma.

There was no other homo or contralateral MRI localization performed as part of the locoregional extension assessment.

Without the help of MammoScreen this subtle distortion could have gone unnoticed. This case clearly demonstrates the effectiveness of MammoScreen and its daily help for junior or experienced radiologists alike.
